Please fill out the following form and send it, along with the Team Fee of $175, to the address listed on the form, or you may pay your Team Fees below by clicking on the Team Fees button below. To make other payment arrangements, contact our Weekend Committee. Be sure to save the dates for Team Meetings which you can find on our Events Page

SPONSORING A CANDIDATE
Before you make the decision to sponsor a Candidate, please read through our Sponsor guidelines and check out more information on our Sponsor Page
Please Note:
This is a 4 part form. The upper left lists who is to fill out each form. All 3 forms, when completely filled out, should be returned together to the Weekend Committee persons listed on the bottom of the form. The 4th page is simply a FAQ sheet for the prospective Candidate and they may keep it.
If there is no Clergy to fill out that page of the form, the application will be accepted without it.
